After joining Optum Care Network–AppleCare through a Medicare Advantage plan, each Optum Care Network member will be visited by a provider who will conduct a health exam, provide guidance and answer questions.


Primary, Specialty and Urgent Care Clinic at Optum–Downey Brookshire

Primary, speciality and urgent care clinic at Optum–Downey Brookshire features all-day walk-ins and is available for your primary care needs when a doctor isn’t available. In addition, we’re supporting key specialties like dermatology, ear, nose and throat and gastroenterology. In addition, we have useful programs such as:


Hospital Discharge Program at Optum–Downey Brookshire

Optum Care Network providers and pharmacists are no site to meet with you and review your hospital stay, medicines and work together to make sure you have everything you need to support your transition home from the hospital.


Coumadin clinic

At our Coumadin clinic, our providers work together with you to monitor your blood thinning medicines and answer questions you have for this treatment.


Activity center

Our activity center is right across the hall from our clinic and offers educational and recreational events on a weekly basis. Visit (events page) for more information. An Optum Care Network pharmacist and dietician each host monthly talks at the activity center to educate patients on how best to care for your health.


Social service programs

Our social services department is available to help you find resources that might be available in the community to help manage your overall health.


Telehealth

We offer discounted rates on Amwell telehealth. This allows you to speak to a doctor from the comfort of your own home using the Amwell app.


Hospital care for end of life

We understand that you may have questions about what hospice care means for you and your loved ones. We’re here to help. Optum Care Network provides emotional, spiritual and clinical support toward the end of life. We’re there to support the whole family. We offer this care whether you’re at home, in an assisted living community, in a skilled nursing facility or in a hospital.

Our hospice care team will work with you and your doctor to develop a care plan that will help you find comfort and preserve dignity — all in a peaceful setting. Our focus is on wellness and quality living. We take care of you by delivering important medications and medical supplies, scheduling your appointments, coordinating your transportation and other social services and arranging anything else you may need 24/7. Most importantly, we’re here to help you establish your health goals and make sure you receive the quality care you need for today and into the future.

Supportive care for you

Health and supportive care is extra care for you at home. Living with a chronic health condition can be tough. Our dedicated supportive care team will work with you and your doctor to make sure you can enjoy your daily activities at home and achieve your health goals.

If you experience any change in your symptoms, you now have 24/7 access to a team who can help you right away. We can also assist you with:

  • Establishing health goals
  • Scheduling doctor appointments and transportation
  • Coordinating home services, including nurse visits, physical therapy, etc.
  • Getting medicines and medical supplies delivered to your home

Care management

Care management is an important service for Optum Care Network members who need a little extra help managing their health care. Our care management team makes sure that you have all of the support you need to stay healthy and get the care you need.

Who we are

Our case management team is made up of experienced nurses and case management assistants:

  • Nurses – Registered nurses (RN) or licensed vocational nurses (LVN) with special training and experience in caring for seniors and members with multiple medical conditions.
  • Care management assistants – Skilled members of the Optum Care Network team who can assist you with finding a specialist, making doctor’s appointments and other needs you may have.

What we do

Your Optum Care Network care management team will work with you, your family and your doctors to develop the care plan that supports your medical and social needs. Some of the services we provide include:

  • Education on disease management and preventive care
  • Education on how to take your medications
  • Help making doctor’s appointments
  • Help getting community resources
  • Advanced care planning

  • Members with complex health care needs or multiple health care issues
  • Members that have gone to the hospital or emergency room multiple times recently
  • Children with complicated health care issues
  • Potential transplant patients (bone marrow, heart, kidney, liver, lung)

Preventive care

Optum Care Network is dedicated to offering the latest in preventive care. Your primary care provider will recommend services to you considering the recommendations provided by the U.S. Preventive Services Task Force.

Center for diabetes care

The center for diabetes care provides Optum Care Network members comprehensive education and individualized support to manage their diabetes. Our mission is to improve quality of life through easy access to quality care and inspired improvement.

We offer all the services you need in one center to make accessing care easy:

  • Medication review and assessment
  • Foot exams and toenail trimming
  • Eye exams
  • Nutrition and exercise counseling

  • May I continue seeing my doctors and specialists?
    Yes. We encourage you to continue seeing your doctors. The diabetes care team will work with your doctors to support your care plan.

  • What should I bring to my first appointment?
    Please bring your glucometer, medications, glucose logbook and anything else you think would be helpful.

  • What should I expect at my first appointment?
    At your first appointment, we’ll be doing a thorough assessment, including an HbA1c measurement on site. Together, we’ll create a plan individualized to your specific needs.
