Meet Your Home Support Navigator

Home Support Navigators work with members and their family to help help enhance their ability to manage their own care.

• Health literacy problems/Struggling to understand medical information
• Managing follow up visits and ensuring continuation of care
• Linkage to community resources to help them manage their chronic diseases (Example: IHSS, Medi-Cal, Meals, Transportation)
• Need assistance with reinforcement of education, home safety assessment, and resource needs
